At the Hughes Family & Friends Foundation, we understand that families face unexpected challenges. Our Direct Support Program is designed to help parents, guardians, caregivers, and teachers in Utah who are supporting children under 18 and experiencing hardship.

We review each application carefully, and every family's story matters to us. If you're unsure whether you qualify, we encourage you to apply. Our team will help determine eligibility.

Application Form

Utility Bill, Lease Agreement, or School Enrollment form. (All personal documents you submit will be used only to determine eligibility for support. They will never be shared, sold, or used for any other purpose.)

Recent Pay Stubs, Tax Return, or SNAP Eligibility Letter. Exceptions can be made for families and children that fall within our foundation's core mission, please don't hesitate to apply if you don't meet the financial requirements but you do meet other requirements.

Copy of the child's birth certificate, court documents showing legal guardianship, school enrollment forms listing the applicant as the emergency contact or guardian, doctor's note or medical record listing the applicant as the guardian, foster care placement documentation.

Recent school enrollment confirmation or student ID with the school's name, letter from school counselor, teacher or principal confirming enrollment, school report card or official correspondence with school letterhead, any document showing the school name and address which can be verified as a Title 1 school.

A short written statement explaining current hardship, a referral letter or statement from a school counselor, teacher, caseworker or healthcare provider describing family's situation, medical bills, recent utility shutoff notice, proof of public assistance.

Food Insecurity; Educational Hardship (lack of supplies or support); Financial or emotional hardship due to medical issues, unemployment etc.
